Her initial social functions were affected: reduced social activities, decreased participation in family roles, and unease about whether she could return to work and maintain relationships with friends and colleagues (Wang et al., 2023). By using unified assessment tools, such as the Cancer Treatment-Breast Function Assessment (FACT-B) and the Anxiety/Depression Scale, the results showed that her scores in social/family well-being, emotional state and daily activity ability were all relatively low. The patient has little confidence in taking care of herself and feels confused about her future life (Tang et al., 2024). She needs comprehensive nursing assistance to support her recovery in physical, psychological and social aspects (Wu et al., 2020; Zhou et al., 2020). 4.2 Intervention implementation The patient adopted a multi-method combined care plan based on wechat. This plan, on the basis of conventional care, also incorporated digital health knowledge explanations, regular follow-ups and interactive support from the nurse team (Wu et al., 2020). The content of physical rehabilitation includes individualized exercise guidance, management of pain and fatigue, and scar care. These contents will be provided through both face-to-face guidance and digital resources. This program also attaches importance to self-care training and physical condition monitoring to promote the recovery of physical functions and enable patients to live more independently (Zhou et al., 2020; Wang et al., 2023). Psychological support is provided through counseling, lectures and group discussions, covering topics such as body image, concerns about recurrence and emotion regulation. Measures to promote social adaptation include family participation, online patient support groups, and training on communication and role re-adaptation. Incorporating gratitude exercises and narrative nursing during the intervention process can enhance patients' sense of hope, adaptability and enthusiasm for participating in social activities (Huang et al., 2024; Xie et al., 2024). 4.3 Result evaluation After 6 months of intervention, the assessment was conducted using FACT-B and related scales (Figure 1) (Wu et al., 2020), the results showed that patients had significant improvements in social/family well-being, emotional state and quality of life. The patient's physical condition has improved, symptoms of anxiety and depression have lessened, confidence in taking care of oneself has increased, and the frequency of participating in family and social activities has also increased. Concerns about cancer recurrence have decreased, and relevant indicators of growth after experiencing the disease, such as personal ability and interpersonal relationships, have also improved (Wang et al., 2023; Xie et al., 2024). These results indicate that a comprehensive, nurse-led and technology-supported intervention approach is of great significance in helping young breast cancer patients reintegrate into society after surgery. This case also emphasizes that in order to enable patients to recover better and maintain long-term health, it is necessary to pay attention to the three aspects of physical, psychological and social simultaneously (Zhou et al., 2020). 5 Nursing Intervention Measures 5.1 Physical rehabilitation intervention Physical rehabilitation is the focus of postoperative care for young breast cancer patients. Planned exercise arrangements, including activity training and gradually enhanced rehabilitation training, have been proven to significantly improve physical condition, reduce fatigue, enhance self-care ability, thereby helping patients resume daily life and social roles more quickly (Zhou et al., 2020). Scar care training and appropriate physical activities can also help prevent complications such as lymphedema and promote the recovery of physical functions (Wei et al., 2024; Wang et al., 2024). Dietary management and lifestyle guidance are also important parts of rehabilitation. Nurses will develop personalized diet plans for patients to help with wound healing, weight control and reduce the side effects of treatment. Explanations of healthy living habits - such as ensuring sleep, relieving stress, and avoiding bad habits - contribute to improving the quality of life and maintaining long-term health (Wang et al., 2023).
