During the emergency phase, rapid first aid response, accurate assessment of the patient’s condition, and coordinated implementation of thrombolytic therapy are essential. Rehabilitation care emphasizes timely interventional treatment, progressive exercise training, psychological counseling services, and the implementation of personalized recovery plans. A scientifically balanced diet plays a critical role in improving patient prognosis, accommodating individual differences, and reducing the occurrence of secondary diseases. The integration of family caregiving and community rehabilitation resources provides strong support for the continuity of care. A systematic nursing model can effectively enhance rehabilitation outcomes, shorten treatment cycles, and manage medical risks. This study aims to highlight the prominent issues in current clinical practice, including the insufficient application of evidence-based nursing and the lack of health education, and proposes improving service quality through strengthened professional training and deeper clinical research. Keywords Stroke care; Acute phase care; Rehabilitation interventions; Psychological support; Family care 1 Introduction This study explores the pathological mechanism and clinical characteristics of cerebrovascular accident (commonly known as stroke). This disorder originates from hypoxic tissue damage caused by cerebral blood circulation disorders. When the blood flow in a local brain area is blocked for more than 5 minutes continuously, irreversible nerve cell necrosis will be triggered (Theofanidis and Gibbon, 2016; Regu, 2019; Rodgers et al., 2021). This disease can cause sequelae such as motor dysfunction, language disorders and cognitive decline, significantly reducing the patients’ ability to live independently and their quality of life. According to the pathogenesis, stroke is mainly divided into two pathological types, 87% of the cases belong to ischemic type, which is caused by cerebral vascular stenosis or thrombosis blockage (Summers et al., 2019; Rodgers et al., 2021; Zhang et al., 2021). The remaining 13% are of the hemorrhagic type, resulting from intracranial hemorrhage caused by damaged cerebral blood vessels. Ischemic types are further subdivided into two types: in situ thrombosis and embolism. As the disease with the second highest disability rate globally, its incidence and prognosis are affected by multiple factors such as geographical distribution, age structure and socioeconomic level (Tulek et al., 2018; Ashcraft et al., 2021). This study will explore a systematic nursing system and integrate a full-cycle management plan for stroke. It will analyze evidence-based nursing interventions during the acute and recovery phases, identify the core techniques for improving patient prognosis, and explain the synergistic effects of multidisciplinary collaborative treatment. The study systematically outlines five stages of stroke management-from pre-hospital emergency care and intensive care to continuous community-based care-forming a comprehensive nursing loop to optimize functional recovery outcomes. 2 Acute Phase Clinical Management 2.1 The core role of emergency response and disease monitoring Pre-hospital emergency care and dynamic assessment are the key points of management during the golden period of stroke treatment.
