International Journal of Clinical Case Reports, 2025, Vol.15, No.1, 44-51 http://medscipublisher.com/index.php/ijccr 50 Future research should focus on expanding the practical applications of community health assessment systems and exploring innovative technologies that can further enhance chronic disease management. Integrating advancements such as telemedicine, artificial intelligence (AI), and mobile health applications can offer real-time data analysis, predictive insights, and improved accessibility for patients and caregivers. Additionally, studying the long-term impacts of these systems on patient health and cost-effectiveness will provide valuable insights into scaling up these models globally. Further exploration into effective strategies for caregiver training and participation, as well as data security measures, will be essential for fostering trust and broad adoption of community health assessment systems. These future developments will help create a more resilient and inclusive healthcare system that better addresses the needs of the aging population.
