This early detection allows for timely interventions, which help in reducing hospital admissions and improving long-term health outcomes for elderly patients (Wu et al., 2021). Regular health assessments contribute to uncovering early signs of diseases like diabetes and hypertension, facilitating proactive treatment and reducing the risk of severe complications (Hernández-Padilla et al., 2023). 3.2 How check-up results support the development of personalized care plans and treatment protocols Results from health assessments conducted through community health systems provide essential data that help healthcare providers design personalized care plans and treatment protocols (Joling et al., 2018).
