Formulating exclusive exercise plans for patients, improving physical therapy methods, and providing personalized health science popularization are the key contents of this management model. Holding regular team seminars and strengthening nursing cooperation can more rationally allocate resources and ensure that patients can receive standardized treatment in a timely manner throughout the rehabilitation stage (Rodrigues et al., 2025). 5.3 Tracking method: digital management and regular assessment Consistent follow-up is the key to early detection of lymphedema, timely treatment and maintaining the rehabilitation effect. Digital medical means such as online follow-up, remote consultation, and wearable monitoring devices can enhance patients' cooperation, facilitate remote understanding of their conditions, and reduce the risk of lymphedema (Marakai et al., 2025; Yao et al., 2025). Through frequent digital intervention, such as regular sending of popular science reminders, video guidance of movements, online consultation, etc., patients will be more willing to adhere to self-care, and arm function can also recover better (Anik et al., 2023; Wu et al., 2025). The content of follow-up should be unified and standardized, such as measuring arm length in a fixed posture, having patients provide feedback using self-assessment tools, and conducting bioimpedance tests, etc. These methods can all help detect mild lymphedema as early as possible and facilitate timely adjustment of the care plan. Combining digital health methods with traditional follow-up can provide a more comprehensive understanding of the patient's recovery status, offer real-time suggestions, and adjust the rehabilitation plan based on the actual situation. This model can not only enhance the therapeutic effect, but also enable patients to be more proactive in long-term health management (Lippi et al., 2023; Liu, 2025).
