This can not only relieve the pressure on the medical system, but also ensure the continuous care of patients, enabling them to receive appropriate care in a timely manner in the community (Eskes et al., 2020). 5 Practical Models and Research Progress 5.1 Nursing staff training intervention: enhance the nursing ability and confidence of family members Nursing training for family members is the key to getting them involved in the post-discharge care of patients, enabling them to learn the knowledge and skills of caring for cancer patients at home. These trainings usually include explaining disease knowledge, teaching practical operation methods, and providing psychological support to family members. Facts have proved that this can reduce the pressure on family members when taking care of patients, enhance their confidence and improve the level of care (Ahn et al., 2020; Samuelsson et al., 2021). The training will also teach family members how to communicate and enhance their feelings, enabling them to better cope with various difficulties encountered during the care process (Ferrell and Wittenberg, 2017). Studies have all shown that this kind of training can enable family members and patients to live better and also reduce the burden of family care (Ahn et al., 2020). However, more rigorous research is still needed to see how the long-term effects are, and these training models should also be extended to more people and different care environments (Ferrell and Wittenberg, 2017; Samuelsson et al., 2021). 5.2 "Internet+nursing" model: achieve remote guidance and health monitoring The "Internet+Nursing" model utilizes online platforms to provide remote assistance, knowledge popularization and health condition monitoring for cancer patients and their families. In this way, patients and their families can communicate with medical staff at any time, facilitating timely treatment of symptoms and receiving continuous guidance. This is particularly useful for caring for patients at home. Online means such as video consultations and online learning materials can make communication among people smoother, family relationships closer, and treatment effects better (Li et al., 2025). The long-term implementation of family-centered online care services has achieved remarkable results in improving the quality of life of patients and their families and accompanying them throughout the entire cancer treatment process (Li et al., 2025). Integrating intelligent technology into the nursing model is a very promising direction, which can make it more convenient for patients to obtain help and ensure that the care after discharge will not be interrupted. 5.3 Multidisciplinary collaborative care: promoting coordinated management between healthcare providers and families The multidisciplinary collaborative nursing model brings together medical staff from different departments to work with patients' families to ensure that cancer care is coordinated and comprehensive. This model can facilitate communication among patients, their families and medical staff, address the concerns of family members, and also enable them to discuss treatment plans together (Dieperink et al., 2020). This cooperative approach is particularly effective when planning the care program in advance, as the participation of family members enables the care program to meet the expectations of patients (Kishino et al., 2022). Studies have pointed out that patients and their families should be cared for as a whole. Nurses play an important role in implementing culturally appropriate and family-centered care measures (Dieperink et al., 2020; Li et al., 2025).
